« Evidence-based treatments for borderline personality disorder: Implementation, Integration, and Stepped Care »


Abstract:


Several manualized psychotherapies for the treatment of borderline personality disorder (BPD) have been validated in randomized controlled trials. Most of these approaches are highly specialized, offering different formulation of BPD and mechanisms by which recovery is made possible. Mental health clinicians are challenged by the degree of specialization and clinical resources these approaches require in their empirically validated adherent forms. While the advent of these effective treatments have provided optimism for the treatment of BPD, clinicians may feel limited in their ability to offer any of them or may integrate an eclectic assortment of features of different treatments. This lecture will evaluate four major evidence based treatments for BPD- dialectical behavioral therapy (DBT), mentalization based treatment (MBT), transference focused psychotherapy (TFP), and general psychiatric management (GPM)- and possible modes of implementation in adherent and integrative forms. Models of implementing these diverse treatment approaches will be evaluated, considering different advantages and cautions about combining evidence based treatments for BPD. A proposal for appropriating care through assessment of clinical severity will be presented in a stepped care model.


Short biography:


Lois Choi-Kain, MEd, MD, has recently transitioned into John Gunderson's former position as the director of the Adult Borderline Center and Training Institute at McLean Hospital. In 2009, she developed McLean Gunderson, an intensive, specialized residential program for adult women with severe personality disorders which uniquely integrates multiple evidence-based treatments for borderline personality disorder (BPD) in a rigorous and scientifically informed way. A few years later, she developed the McLean BPD Training Institute (BPDTI), which is a major center for teaching evidence based approaches to treating severe personality disorders including MBT, General Psychiatric Management (GPM), and Transference Focused Psychotherapy (TFP). In addition, as an assistant professor of psychiatry at Harvard Medical School, Dr. Choi-Kain actively conducts research and publishes papers on BPD and its evidence based treatments. Her areas of specialization include attachment, cognition, personality disorders, and evidence based psychotherapies.
